Scholarship Terms and Conditions - AVA Annual Scientific Meeting 2025

1. Eligibility

1.1. The scholarship is awarded to support attendance at the Association for Vascular Access Annual Scientific Meeting, taking place September 19-21, 2025, in Kissimmee, Florida. Scholarship recipients are expected to attend the event in full, including general sessions, educational tracks, and applicable networking events.

1.2. Applicants must be 18 years or older at the time of entry.

1.3. Applicants must currently be involved in clinical practice with responsibilities related to vascular access and must reside in the United States. Preference may be given to applicants who demonstrate need, leadership, and a commitment to improving vascular access practices.

1.4. Employees of Eloquest Healthcare, its affiliates, and their immediate family members are not eligible to participate.

2. Application Process

2.1. Applicants must complete the official application form available on the Eloquest Healthcare website.

2.2. All applications must be submitted by July 18, 2025, at 11:59 PM ET. Incomplete or late applications will not be considered.

3. Selection Process

3.1. Winners will be selected based by a panel of reviewed based on the strength of the application responses. All decisions are final.

4. Prize

4.1. Four scholarships will be awarded, each valued at $2500.

4.2. The scholarship covers conference registration fees, travel expenses, and accommodations.

4.3. Any additional expenses incurred beyond the $2500 prize value are the responsibility of the winner.

5. Notification of Winners

5.1. Winners will be notified via email and/or phone by July 25, 2025.

5.2. If a winner does not respond within 7 days of notification, they forfeit the prize, and an alternate winner will be selected.

6. Acceptance of Prize

6.1. Winners must accept the prize as awarded; no substitutions will be allowed.

6.2. The prize is non-transferable and non-refundable.

6.3. Winners should check with their tax advisor to determine the tax treatment of this prize.

7. Withdrawal and Reimbursement

7.1 In the event a scholarship recipient is unable to attend the AVA 2025 conference, they must notify the Eloquest Healthcare immediately and return the full scholarship amount.

8. Conditions of Participation

8.1. Winners agree to allow Eloquest Healthcare to use their name, photo, and application details for promotional purposes.

9. Liability

9.1. Eloquest Healthcare is not responsible for any injury, loss, or damage of any kind resulting from participation in the scholarship or from acceptance or use of the prize.

9.2. Winners assume all liability for any injury or damage caused, or claimed to be caused, by participation in this scholarship or use or redemption of any prize.

10. General Conditions

10.1. Eloquest Healthcare reserves the right to cancel, suspend, and/or modify the scholarship, or any part of it, if any fraud, technical failures, or any other factor beyond Eloquest Healthcare's reasonable control impairs the integrity or proper functioning of the scholarship.

10.2. Eloquest Healthcare reserves the right to disqualify any individual it finds to be tampering with the application process or the operation of the scholarship or to be acting in violation of these Terms and Conditions.

11. Governing Law

11.1. The scholarship and these Terms and Conditions are governed by the laws of the State of Michigan and any disputes will be subject to the exclusive jurisdiction of the courts of the State of Michigan.

By participating in this scholarship, you acknowledge that you have read, understood, and agree to be bound by these Terms and Conditions.
